Teacher Evaluation Forms:

Teacher Evaluation Forms: Used to evaluate a Teacher’s class content.

Student Incentive: Students receive a $5 credit toward their CRA membership fee. This credit expires.

Confidentiality: Must be completed at home in privacy and away from the Teacher. Forms are viewed only by the CRA Board; they are never shared with the teacher (even if the teacher is a Board Member).

Submission: Students must submit the forms directly; teachers cannot submit them. If submitted by the Teacher, they will not be accepted.

Expiry: The $5 membership credit is granted upon class completion and if the Teacher Evaluation Form was completed. It expires 6 months after the final class date, and applies only to applications made within this 6 month window.
